Lesson 16 Creating an Action Plan
Once the team agrees on the question to focus on, work together to create a simple action plan.
Use the Team Health Action Plan template or capture ideas using Post‑it notes. Download if you haven’t already done so from Lesson 15.
Step 1: Select the focus question
Choose ONE of the 12 questions to focus on as a team.
Step 2: Define the question.
Ask the team: “What does it look and feel like when this item is working perfectly?”
Step 3: Identify Builders and Breakers
Ask the team:
What positively influences or builds this question for us?
What gets in the way or breaks this question for us?
Step 4: Identify who can help
If your team has completed CliftonStrengths, review the Using Strengths Guide.
Different strengths notice different problems and contribute different perspectives. Consider which people in your team may naturally help improve this area.
The guide identifies the strengths most likely to contribute to each Team Health Check question and provides a practical coaching question to help start the conversation.
Step 5: Identify Actions
Ask the team:
What is one action we should start doing?
What is one action we should stop doing? This is to create space for our new action
What is one action should we continue doing?
Step 6: Follow up
Ask the team:
Who will lead each action?
When and how will we check progress?
Should this become a standing agenda item in team meetings?
When we succeed, how should we recognise and celebrate?
Record the outcomes in the Action Plan template so the team can track progress.
